If you would like to have Dr. Fahim become your family Doctor please first
book an appointment at the Flin Flon Clinic and see him there to get it all set
up.

E.R.I.K. ( Emergency Response Information Kit)

E.R.I.K. ( Emergency Response Information Kit)  is now available at the Child/Family Resource Center
The kit is a clear plastic magnetized sleeve which you put on your refrigerator,
and it includes;Medication and Medical Information Sheet Health Care Directive
Organ and Tissue Donor Card
Once you have completed all the information, there is
a sticker to be placed on our door which alerts emergecy personnel that you have
the E.R.I.K.
